Our Short-Term Care Services

While working through physical or post-operative challenges, guests enjoy customized therapy in comfortable and finely appointed accommodations. We handle all of the “paperwork” for our guests and their surgeons by working with Medicare and insurance carriers, so there’s one less thing to worry about. 

Additionally, Montowese Center for Health and Rehabilitation invests in the highest clinical oversight—pairing Nurse Practitioners (NPs) with technology—because we recognize that everyone benefits when our patients receive the best care.  

Physical, Speech, and Occupational Therapy

Physical therapy focuses on the identification of physical impairments and pain, allowing our guests to achieve their goals of independence. Physical therapies include:

  • Mobility retraining
  • Balance restoration
  • Therapeutic exercises
  • Physical conditioning
  • Neuromuscular retraining

Occupational therapy focuses on improving independence and guests’ ability to comfortably complete Activities of Daily Living (ADLs), which include feeding, dressing, and grooming, as well as daily tasks such as preparing meals and even caring for a pet.

Speech therapy focuses on communication, swallowing, and cognition, which is particularly important for those affected by strokes, respiratory illnesses, brain injuries, or dementias.

Heart Failure and Cardiac Recovery

We tailor our Heart Failure services through an interdisciplinary approach to the needs of each patient, providing education and support around lifestyle changes. During a patient’s stay, we help them manage their medications and monitor their recovery by offering:

  • American Heart Association guidelines are followed for diagnosis and management of symptoms and treatment
  • On-site physicians, emergency equipment, and trained staff
  • Counseling for lifestyle changes and smoking cessation

Pulmonary Disease Management

Pulmonary Disease Management helps patients recover through an interdisciplinary approach, including treatment, education, and ongoing monitoring of the individual’s progress. We may offer:

  • On-site respiratory therapists
  • Tracheotomy and tracheostomy care programs

    • COPD
    • CPAP / BIPAP
    • Ventilator dependency

Neurology and Stroke Rehabilitation

Neurology and stroke rehabilitation services help patients recover from their stroke event and use a range of therapy and treatments to improve their functional abilities. Clinicians and skilled nurses oversee each patient’s recovery alongside an interdisciplinary approach, including education and assistance with activities of daily living (ADLs).

  • Post-stroke rehabilitation

Orthopedic Injuries and Post-Surgical Care

Our Orthopedic Injuries and Post-Surgical Care services help patients recover from injuries and operations through guided treatment and therapy from skilled clinicians and specialists.

Diabetes Management

Diabetes Management helps prevent further decline in patients diagnosed with Chronic Kidney Disease (CKD). With early detection, it is possible to slow, stop, or even reverse CKD, depending on its cause.

  • Routine medical management of associated comorbidities

Wound Care

For patients with wounds, our preventative and advanced wound therapies are critical to their recovery and comfort.

  • Lower Extremity Neuropathic Disease (LEND) and Diabetic Foot Ulcers (DFUs) management

Amputee Rehabilitation

Amputee Rehabilitation maximizes the functional abilities of individuals with limb loss to achieve their maximum potential to lead full, productive lives. We offer:

  • Pre-prosthetic training, including pain control, training, and wound healing
  • Prosthetic training, including gait and functional mobility training, and donning/doffing of prosthetics
  • Emotional support from psychologists, social workers, and patient and family support groups

IV Therapies

Intravenous therapy (IV) is a way to give fluids, medications, nutrition, or blood directly into the bloodstream through a vein. We offer:

  • IV Lasix
  • IV Solumedrol
  • Central lines

Advanced Capabilities and Programs

Our Advanced Capabilities and Programs are led by accredited providers to give patients access to enhanced clinical specialties. Select programs feature a series of measurable patient outcomes designed to exceed our standards of providing excellent care.

Advanced Cardiac Capabilities

As an expansion of our heart failure and cardiac recovery services, our advanced cardiac capabilities may include:

Advanced Pulmonary Capabilities

Advanced IV Therapies

As an expansion of our IV therapy services, we offer Total Parenteral Nutrition (TPN), a method of feeding that bypasses the gastrointestinal tract. A special formula given through a vein provides most of the nutrients a person needs. This method is used when someone can’t or shouldn’t receive feeding or fluids by mouth.

Peritoneal Dialysis

Peritoneal Dialysis (PD) is an alternative to hemodialysis (HD) for patients requiring renal replacement therapy. With PD, patients receive treatment throughout the night and can be taught to conveniently use the equipment independently. Nighttime therapy allows guests to enjoy their days participating in therapeutic or recreational activities.

Acquired Brain Injury Program

This program specializes in caring for people who have experienced a variety of brain injuries and is tailored to their individual abilities so they can meet their personal goals. Our program may include:

  • Physician-directed care
  • Physical, occupational, speech, and respiratory therapy
  • On-site neuropsychology, psychiatry, and pulmonology services
  • Family support services and education

Parkinson’s and Movement Disorders Program

As an expansion of our neurological and stroke rehabilitation services, this program starts with a comprehensive rehabilitation assessment and includes a tailored approach to improving the quality of life for residents diagnosed with Parkinson’s Disease or Parkinson’s Plus Syndrome. Program services may include:

  • Medication management
  • Behavioral health supportive services
  • Physical, occupational, and speech therapy
  • Rehabilitation treatments are focused on:
    • Movement patterns
    • Gait and functional mobility training
    • Activities of daily living training
    • Balance and postural training 
    • Cognition and memory exercises

Mental Health & Life Enrichment Program

As part of helping our residents feel empowered and enhance their self-determination, this program focuses on a relaxed, open environment where residents enjoy therapy and behavioral management while addressing their needs for clinical support and more specialized therapies. Our services may include:

  • Dedicated Licensed Professional Social Workers
  • Rounding psychology APRN
  • Physician oversight by psychiatrist and psychologist
  • Six-day-a-week programming
  • Group and individual therapy
  • Weekend groups led by social worker
  • Enclosed walking courtyard
  • Introductory reduced rate for respite stays

Criteria for acceptance:

  • Age 50+
  • Mild to moderate behaviors
  • No physical aggression
  • Medically frail
  • 1 or 2 psychiatric diagnoses, but stable
  • Able to participate in programming
  • No TBIs
  • No sex offenders

On-Site Outpatient Therapy

Our advanced outpatient therapy services help residents meet personalized goals through physical, occupational, and speech therapy. A variety of therapy and treatment options help individuals recover from procedures, treat chronic conditions, or improve their mobility and physical wellbeing.

Long-Term Care Services

As part of our continuum of care, guests can expect a skilled interdisciplinary team to instill a sense of love for life at every stage during their stay.

All of National’s centers offer comprehensive long-term care services, including:

Alzheimer’s, Dementia, and Memory Care

Respite Care

An individual generally cared for at home or in a similar setting can be admitted to our center for a short-term stay to provide caregivers a chance to recover from the stresses of daily care, take a vacation, or address other personal needs. Services include:

  • 24-hour skilled nursing care as needed
  • Medication management
  • Recreation programs
  • Physical and/or occupational therapy
  • Nutritional needs management
  • Support groups for patients and family members, led by licensed social workers

Palliative Care

Palliative Care is a specialized form of health care used to support individuals facing progressive illness. Our Palliative Care Program provides symptom management while treating the whole person and allowing patients the choice of receiving the highest quality of care in their homes. This program is delivered by a dedicated team including hospice-trained nurses, physical therapists, home health aids, social workers, and volunteers. Services and outcomes include:

  • Improving the ability to continue participating in day-to-day activities
  • Achieving the best possible quality of life
  • Managing pain and symptom control
  • Offering a support system to help patients, families, and/or caregivers cope
  • Helping identify treatment options and care goals
  • Guiding the patient through the health care system
  • Assisting with advance care planning, financial issues related to illness and caregiving responsibilities, and insurance and financial paperwork

Hospice Care

Our Hospice Program addresses an individual’s physical, emotional, social, and spiritual needs while attending to the equally important needs of their family and caregivers. Our team of hospice professionals and volunteers work cooperatively with the individual, their family, and primary care physician to create a customized care plan involving:

  • Registered nurses to address physical needs, such as pain and symptom control
  • Social workers to address psychosocial, emotional, and financial needs
  • Religious and spiritual counselors
  • Hospice aides assist with personal care and household tasks
  • Volunteers support the patient and family
  • Respite care is available to allow primary caregivers time to rest
  • Medical equipment and supplies are coordinated as necessary
  • Medications are provided and managed with insurance providers
  • Bereavement counselors to help family members and loved ones

Ventilator Care (coming soon)

Our Ventilator Care program takes an aggressive multi-disciplinary approach to ventilator rehabilitation. Ventilator weaning reduces or eliminates dependency on a ventilator for breathing. Clinical expertise allows us to design an individualized respiratory health plan incorporating physical, occupational, and speech therapies. For those requiring chronic ventilator services, our program is designed to provide optimum care and comfort. Services include:

  • Four levels of care: ventilator, tracheostomy, short-term, and long-term pulmonary
  • Respiratory therapist on staff 24 hours per day, 7 days per week
  • On-site pulmonologist
  • “Out of Bed” philosophy for patients to be out of their room for meals, therapy, and other activities
  • Support groups for patients and family members, led by licensed social workers
  • A separate unit on campus with a therapy gym, dining rooms, Wi-Fi, and lounge areas
